• On the homepage, go to the "Admissions" tab.
• To fill out the Montessori-1 registration form, click on the "Apply" tab.
• The registration form will be displayed on the screen.
• Fill out the form and submit it along with the registration fee.
• After submitting, print out two copies of the filled online registration form and the receipt of payment. One copy must be submitted to the school along with the required documents, while parents can keep the other copy for furture reference.

Documents Required for Notre Dame Academy, Jamalpur, Admission to Montessori-1

• Birth certificate from the hospital
• Birth certificate from the municipality/panchayat
• For Christians, a Baptismal certificate from the parish church is required
• Voter ID and Aadhaar card of both parents
• An affidavit in English by the First Class Executive Magistrate/Notary, of the child’s name, date of birth, with the mother’s & father’s names.
  • In future, the date of birth and the name of the child will not be changed.
  • The date of birth should be written in words also.
  • The affidavit will have the signatures of both parents.
  • In future, the names of the parents will not be changed.
• Photo copy of the October / November electricity bill of the previous year.
• Those who are residing in the railway quarters, must submit the quarter allotment letter with quarter no. and photocopy of ID card of the employee instead of the electricity bill.
• A recent photograph of the child with father and mother is to be pasted in the given space in the form.
