Begin by downloading the application form from the official CBSE website: http://cbse.nic.in/faq/dupform.pdf. This form might be available online or at the CBSE office.
Read the instructions at the bottom of the application form thoroughly. Fill out all details accurately, ensuring they match the desired correction.
To verify the request, students need to submit specific documents. These include school leaving certificates and admission documents. Prepare an affidavit on stamped paper, sworn before a notary public or magistrate, stating the correct name or DOB and the reason for the correction.
Submit a copy of the birth certificate issued by the concerned authorities. Provide a copy of school leaving certificate. Also, include a copy of your identity proof, such as an Aadhaar card, passport, or driving license.
Attach a copy of the mark sheet with the incorrect information. Send these documents along with the completed application form to your school. They will then forward them to the CBSE board on your behalf.
There are processing fees associated with CBSE name and DOB corrections. The amount varies depending on the nature of the change. Check the fee structure here:
Name Correction in Mark sheet/Certificate: Rs. 1000 (Inclusive of mark sheet/certificate fee)
Change in Parent's Name: Rs. 1000 (Plus actual cost of the document)
Migration Certificate: Rs. 250
Provisional Certificate: Rs. 200
Review: The CBSE will review the application and documents to verify the accuracy of the information.
Approval: If the application is approved, the board will initiate the process of correcting the name or DOB in the records.
New Mark Sheet: Once the correction is made, the CBSE will issue a revised mark sheet with the correct information.
Certificate: If necessary, a corrected certificate will also be issued.
To accommodate name or surname changes, the CBSE will only consider applications that have been officially approved by a Court of Law and subsequently published in a government gazette.
If the change is approved, the board will initiate the process of updating the student's records. They will need to provide documentation confirming the court order and its publication in the official gazette.
The CBSE allows for corrections in names, surnames, or parent names on mark sheets if the errors are due to spelling mistakes or typographical errors. These corrections must align with the information recorded in the school's records or the list of candidates (LOC) submitted to the CBSE.
Attach the following attested documents:
A true copy of the admission form(s)
A true copy of the school leaving certificate from the previous school
A true copy of the relevant page from the school's admission and withdrawal register
The head of the institution will verify the information and forward the application to the CBSE for processing.

The Medhavi National Scholarship Program, under the Human Resources & Development Mission (HRDM), offers financial assistance to meritorious students through a scholarship exam. If you have uploaded screenshot of your 12th board result taken from CBSE official website,there won,t be a problem with that.If the screenshot that you have uploaded is clear and legible. It should display your name, roll number, marks obtained, and any other relevant details in a readable forma.ALSO, the screenshot clearly show it is from the official CBSE results portal.
