Children of COVID-19 Affected/Deceased Parents (March 1, 2020 - March 31, 2022)

A document certified by a competent authority confirming the death due to COVID-19.

Issued by the respective authority for Scheduled Caste/Scheduled Tribe children, including sub-divisional officer/tehsildar/district collector's certified document.

Certified by a civil surgeon/medical officer.

Certified document confirming death due to COVID-19 from a recognized authority, valid death certificate, or Form No. 4.

Proof of annual income from the employer, such as salary slips for the financial years 2022-2023 and 2023-2024.
